Company Overview

Care Continuity (CC) delivers a technology-enabled care concierge and patient navigation workflow platform with complementary services model. CC delivers the “last mile” care coordination and logistics that are often overlooked and result in many health care providers and health plans underserving their communities and populations that they manage (directly impacting payment and revenue share). 


The Company’s hospital and health plan customers invest heavily in developing their provider networks and then have not historically connected this “network” for patients, caregivers and network sponsors.

Unique Perspective + Engagement

Empactful Capital met their founder/CEO in early 2019 and the formally engaged with Care Continuity in May 2019. This consultative engagement included strategic planning, growth plan execution, go-to-market plan development and target market expansion, and team and organizational development.


Empactful’s subject matter experts (SMEs) have deep expertise in provider network management, value-based care enablement and member / patient engagement. This expertise was pivotal in our engagement and developing our investment thesis.

Company / Engagement Highlights

Care coordination is a top priority for mental and physical health providers and payors; effectively managing these populations (often with multiple co-morbidities) requires care logistics between providers and across network(s).


Growth Plan Results: 


  • Revenue Growth: Revenues have grown 2x for 2 years (2021 and 2022) post the engagement and has reached early growth stage revenue scale.  
  • Contract Bookings: Company has achieved record levels of new contract bookings driven by significant customer expansion and new customer deals.
  • Team Development: Empactful recruited the Company’s COO (Q3 2020), CEO (Q4 2021) and 3 key industry leaders to Board roles (Q2 2021).
  • Operational Discipline: Defined and implemented operating metrics and key growth milestones leveraging Empactful’s best practice frameworks.
  • Growth Funding: During 2020 and 1H 2021, Empactful prepared the Company for its Series A growth round that Empactful led in Q2 '21 to rapidly scale revenues.
